Players start by infecting "Patient Zero" with a pathogen of their choice, ranging from bacteria and viruses to more exotic options like bioweapons and mind-controlling parasites. The goal is to carefully evolve the pathogen, balancing between increasing infectivity to spread it globally and increasing lethality to kill off the population.

The different disease types available each require unique approaches, as bacterial plagues, viral outbreaks, and mind-controlling parasites all have their own strengths and weaknesses that dictate how they should be evolved. The 23 unique gameplay scenarios introduced in the game add additional layers of challenge, presenting varying starting conditions that force players to adapt their pathogen strategy. The game also features a robust set of difficulty levels, from the introductory "Casual" mode all the way up to the punishingly difficult "Mega-Brutal" setting. Higher difficulty levels force players to be more strategic in their pathogen evolution, as governments and the scientific community become faster and more effective at developing cures.
