Players control a heavy metal band on a road trip through the zombie apocalypse, with the core gameplay loop involving hitting bass drum notes in time with the music to fire the band's vehicle-mounted guns at hordes of zombies. As players build up a combo by hitting notes accurately, their firepower upgrades from a pistol to more powerful weapons like shotguns and cannons. Missing too many notes causes the player's guns to jam, leaving them vulnerable. Higher difficulty levels add snare drums that fire grenades and cymbals that provide additional firepower. Mastering the timing and coordination required to hit all these different drum notes is the core challenge.

During certain segments, usually boss battles, the game tasks players with steering the car up and down to avoid enemy attacks while still maintaining their drum inputs.
