This deliberately complicated control scheme transforms surgical procedures into physics-based comedy.
The experience transports players through wild surgical scenarios: sterile operating rooms, violently bumping ambulance interiors, zero-gravity space stations, and bizarre alien medical settings. Surgical options include heart transplants using comically inappropriate tools, kidney replacements with unpredictable outcomes, brain surgeries that might involve more hammer than precision, and eye and tooth procedures added in later updates.
Absurdity drives the core experience. Players can accidentally replace organs with random objects, use a hammer instead of a scalpel, inject themselves with mysterious medical substances, and trigger chaotic environmental interactions.
