Players must explore the house, find and use items to unlock the main door or alternative escape routes like a boat or helicopter. However, the house is patrolled by the elderly couple - Granny and Grandpa - who will relentlessly chase and attack the player if they are spotted. This creates a tense, high-stakes gameplay loop of searching for items, avoiding detection, and making daring escapes to escape within the five in-game days.

Compared to the first game, this sequel introduces significantly more challenge and danger. Grandpa, a new antagonist, is nearly deaf but will charge towards any noises he does hear, while Granny maintains her sharp senses from the previous game. Additionally, a creepy, crawling creature in the backyard further threatens the player's survival.
